Thank you for your interest in volunteering at the YMCA of North Central Ohio! Volunteers are vital to the YMCA. Without them, we wouldn't be able to meet the needs of the kids, families, and adults who live in Richland County.

At the YMCA, we know that your time and talent are precious, and we want every minute you spend with us to be worthwhile. That's why we're asking you to take a few minutes to fill out this application. It will help us begin to make the right match between your skills and interests and the opportunities available.
You will find questions on this form that are required to do a background check (background checks are FREE of charge). We hope you'll understand that, unfortunately, there are a few people who apply for volunteer jobs at the YMCA for the wrong reasons. The YMCA, however, makes an active effort to prevent abuse. So, even though we may know you well, we reserve the right to conduct background on all volunteers. It's just one of the many ways we help protect children and other vulnerable people the YMCA serves. Thanks for your cooperation in this effort.

The YMCA of North Central Ohio requires all volunteers to complete a Conviction/Non-Conviction Statement. A criminal background check consisting of a statewide criminal search and a nationwide sex offender search will be conducted. By completing Section A or B below and signing this form, the volunteer gives the YMCA of North Central Ohio authorization to run both searches.
Section A | Non-Conviction statement

I hereby attest that I have never been convicted of or pleaded guilty to crimes set forth in division (A) (8) or (A) (9) of section 109.572 or division (A) (1) of section 4105.09 (see Prohibited Offenses list here) of the Ohio Revised Code or an existing or former offense of any municipal corporation, this state, or any other state that is substantially equivalent to any of these offenses.
I attest that no child has been removed from my home as described in section 2151.353 of the Revised Code.
Section B | Conviction statement
I hereby attest that I have been convicted of a crime set forth in division (A) (8) or (A) (9) of section 109.572 or division (A) (1) of section 4105.09 (see Prohibited Offenses list here) of the Ohio Revised Code or an existing or former offense of any municipal corporation, this state, or any other state that is substantially equivalent to any of these offenses.

I hereby affirm that the information given by me on this application for volunteering is complete and accurate. I understand that any falsification or omission will be immediate grounds for dismissal. I authorize investigation of all statements contained in this application. I understand this agreement and have read it carefully before signing. I acknowledge and accept that all volunteers are subject to criminal history checks & sexual offender registry checks. I hereby authorize the obtaining of consumer reports or investigative reports, aka background checks, by the YMCA of North Central Ohio at any time after the receipt of this authorization and throughout my volunteer time if applicable. I understand that I am expected to conduct myself in a manner that exhibits the YMCA's core values of honesty, caring, respect & responsibility and will be held responsible for my behavior & conduct.
